Mock Question: What Happens to Your Blood Pressure During Exercise?

You may have learned blood pressure for your Level 3 exam, but could you explain what actually happens to it during exercise?

Your mock question

During dynamic aerobic exercise, what happens to systolic and diastolic blood pressure?

A. Both increase significantly
B. Systolic increases while diastolic stays similar or may fall slightly
C. Systolic falls while diastolic increases
D. Both stay exactly the same

The answer is B.

But knowing the answer isn’t enough.

You need to understand why.

Why do you need to know about blood pressure during exercise?

As a personal trainer, you’ll work with people whose cardiovascular system is constantly responding to exercise.

Understanding blood pressure helps you explain what is happening inside the body.

It also helps you recognise when a response is expected and when something may need attention.

First, what do systolic and diastolic mean?

Systolic blood pressure is the pressure in the arteries when the ventricles contract.

Diastolic blood pressure is the pressure when the ventricles relax.

So when you see a reading such as 120/80 mmHg:

120 = systolic

80 = diastolic

Simple enough.

But exercise changes the picture.

What happens to blood pressure during exercise?

During dynamic aerobic exercise, your muscles need more blood.

Your heart responds by beating faster and pumping more blood with each beat.

This increases cardiac output.

Remember:

Cardiac output = heart rate × stroke volume

As exercise intensity increases, cardiac output usually increases too.

This helps drive the rise in systolic blood pressure.

At the same time, blood vessels supplying the working muscles vasodilate.

This means they widen.

This helps more blood reach the muscles and reduces resistance in those areas.

So you have two things happening at once:

Cardiac output ↑

Blood flow to working muscles ↑

And your body constantly adjusts blood pressure to keep blood flowing where it is needed.

The bit learners often struggle with

If blood vessels are widening, why doesn’t blood pressure simply fall?

Because blood pressure is influenced by more than one factor.

The increase in cardiac output helps raise arterial pressure.

At the same time, vasodilation in working muscles helps reduce resistance.

The body balances these changes.

That is the important bit to understand.

What about blood pressure during resistance training?

Heavy resistance exercise is different.

When you perform a hard lift, contracting muscles can compress blood vessels.

This can cause a much larger temporary rise in blood pressure.

And if someone holds their breath during a heavy lift, the Valsalva manoeuvre can create large changes in pressure inside the chest and affect the cardiovascular response.

This is one reason appropriate breathing matters during resistance training.

What happens to blood pressure AFTER exercise?

Your heart rate and cardiac output begin to fall.

But the blood vessels in the working muscles may remain dilated for a while.

And the muscle pump that helped return blood towards the heart is no longer working in the same way.

Blood pressure therefore has to readjust.

This is one reason a gradual cool-down can be useful after vigorous exercise.

It gives the cardiovascular system time to adjust rather than stopping suddenly.

Cardiac output ↑

More blood reaches working muscles

Working muscles vasodilate

Blood pressure is regulated

That’s the A&P you need to understand.

Not just:

“Blood pressure increases during exercise.”

Your practical PT application

Imagine you’re coaching a client through a challenging workout.

Their heart rate rises,

breathing increases.

systolic blood pressure rises.

their working muscles receive more blood.

These aren’t separate events.

They’re all part of the body’s response to exercise.

As a PT, understanding that connection helps you make better decisions about exercise intensity, recovery and client safety.

And remember: unusual symptoms or responses shouldn’t simply be dismissed as “part of exercise”. Your role is to recognise when something isn’t expected and respond within your scope of practice.

Common exam mistake

“Blood pressure increases because the heart pumps harder.”

That’s only part of the answer.

A stronger Level 3 answer connects:

cardiac output + blood vessel diameter + peripheral resistance + blood flow.

That’s the understanding exam questions are testing.
